Periodontal disease is a chronic infection of the tissues that support the teeth: the gum, the periodontal ligament and the alveolar bone. It usually starts as gingivitis, an inflammation of the gums caused by plaque buildup. If gingivitis is not treated in time, the infection spreads deeply, forming periodontal pockets between the gum and the tooth, and progressively attacking the bone that supports the tooth.
Periodontitis is the second cause of tooth loss after tooth decay, according to data published in Journal of Dental Research. Stage 1: Full diagnosis and assessment
At the first consultation, the doctor performs a complete periodontal examination: measuring the depth of the periodontal pockets with the periodontal probe, assessing bleeding on probing, checking tooth mobility and analyzing the digital X-ray taken directly in our office. The X-ray shows us the level of the alveolar bone and the presence of possible bone defects. Based on this data, we determine the stage of the disease (gingivitis, early, moderate or advanced periodontitis) and develop the treatment plan.
Stage 2: Professional sanitation and descaling
The first therapeutic intervention is the removal of tartar (calculus) from the surface of the teeth and below the gum line. Tartar is the mineralized bacterial plaque that cannot be removed by brushing and that sustains chronic inflammation. Professional scaling, combined with professional brushing and personalized hygiene instructions, is the basis of any periodontal treatment. Without rigorous hygiene at home, no professional treatment can give stable results. Stage 3: Periodontal treatment with Er:YAG laser
For medium and deep periodontal pockets, we use the Er:YAG laser as an integral part of the treatment protocol. The laser is effective in several ways: it cleans the infected tissue inside the periodontal pockets, removes subgingival tartar, decontaminates the surface of the tooth root, and removes bacteria from areas hard to reach with classic mechanical instruments.
Unlike classical curettage with mechanical instruments, the Er:YAG laser produces less discomfort during the procedure, less bleeding and faster healing of the gingival tissues. The laser does not affect the healthy periodontal ligament, intact bone tissue or healthy gums in the vicinity, which makes the treatment more conservative and gentler on the structures we want to preserve. In addition, laser energy promotes tissue biostimulation and regeneration, which contributes to faster recovery.
The study published by Aoki et al. (2023) in Journal of Periodontology demonstrated that Er:YAG laser-assisted periodontal therapy produced a significantly greater reduction in periodontal pocket depth compared to mechanical treatment alone (2.78 mm vs. 1.89 mm at 12 months, p=0.012).
Stage 4: Reassessment and maintenance plan
6-8 weeks after the active treatment, the doctor re-evaluates the periodontal condition: re-measures the depth of the pockets, checks the bleeding on probing and compares with the initial values. Based on this reassessment, the maintenance plan is established: the frequency of professional hygiene sessions (usually every 3, 4 or 6 months, depending on the severity) and periodic periodontal checks.
Maintenance is the most important part of periodontal treatment. Without it, the disease returns. The treatment becomes more complex: in addition to descaling, the Er:YAG laser allows accessing and decontamination of medium-depth pockets minimally invasively, reducing in many cases the need for classic periodontal flap surgery. The laser cleans the infected tissue, removes subgingival tartar and removes bacteria from areas that mechanical instruments have difficulty reaching.

Advanced periodontitis
Pockets exceed 7 mm, bone loss is severe, tooth mobility is pronounced, and the individual prognosis of affected teeth varies. At this stage, the goal of treatment is to save the teeth that can be saved and to stabilize the situation to prevent further loss.
The Er:YAG laser remains part of the protocol in advanced cases as well: it is used to decontaminate deep pockets, to prepare tissues before possible surgical procedures and to reduce the bacterial load in the areas that will receive implants.
Some teeth may require extraction, in which case our team proposes replacement solutions (dental implant, fixed prostheses or All-on-4/6/8 rehabilitations). Even in advanced cases, periodontal treatment is essential before any prosthetic solution, as implants placed in an uncontrolled periodontal environment have an increased risk of failure.
